DYSLEXIC MIND ASSESSMENTS
Terms & Conditions
It is crucial for a dyslexia assessor to establish clear expectations, ensuring mutual understanding between Dyslexic Mind Assessments and the client regarding assessment procedures, confidentiality, and responsibilities. They safeguard both parties' rights, outline the assessment process comprehensively, and maintain professional standards, fostering a transparent and effective evaluation environment.
Booking
Parents will be sent an assessment booking form and an invoice, once a date and time have been agreed on.
Cancellation
If an appointment is cancelled 14 days prior to the assessment, 50% of the assessment fee will be payable to Dyslexic Mind Assessments. However, no fee will apply, if a replacement assessment is booked by another client. In addition, if an appointment is cancelled 48h prior to the assessment, 100% of the assessment fee will be payable to Dyslexic Mind Assessments. In the event of a cancellation of a booked assessment by Dyslexic Mind Assessments due to weather conditions, illness or similar emergency situations, I reserve the right to move the assessment to a mutually convenient dater and time. A cancellation should be advised via email (ieva.auksakyte@gmail.com) or telephone (07857118435).
Rescheduling
In the event of unforeseen circumstances, the Dyslexic Mind Assessments reserve the right to reschedule an assessment. Any changes to the schedule will be communicated promptly to the students and parents via email or phone. I aim to accommodate rescheduling requests wherever possible, provided advance notice is given and subject to availability. Dyslexic Mind Assessments appreciates your understanding and cooperation regarding any necessary adjustments to the timetable.
Pre-Assessment
Before the assessment date, parents will receive a parent questionnaire and a Visual Difficulties Screening to fill out at their convenience. These forms should be sent back to the assessor no later than 5 days before the scheduled assessment day. This helps ensure a comprehensive evaluation process.
During Assessment
Throughout the assessment, students may have water with them and will have regular breaks between assessments. To facilitate the best concentration, parents are not permitted in the assessment room while their child is undergoing evaluations. This helps create an optimal environment for focused assessment sessions.
Post-Assessment
Following the assessment, parents are kindly requested to settle the diagnostic assessment fee. Once payment is received, the process of compiling the report will begin and is estimated to take up to 14 days. Upon completion, parents will be promptly contacted via email and provided with a copy of the comprehensive report. A follow-up meeting is offered to discuss the report and answer any questions.
Fees
A deposit of £100 should be payed on the day of the booking. The remaining amount must be payed on the day of the assessment. Report writing will not begin until the full amount for the diagnostic assessment is received. If the full amount is not payed on the day of assessment, Dyslexic Mind Assessments will require a fee of £250 to pay for pre-assessments work.
Communication
Dyslexic Mind Assessments are open for clients 9am-5pm Monday-Saturday. Contact should be made via the website, email or telephone.
